SmartCare Palm Harbor has the following financial policies in place to help with your financial obligations.
Please remember to bring your insurance card
Uninsured and/or cash patients will be required to pay for services on or before their admission date
Patients with verified insurance will be required to pay their estimated portion ONLY, on or before the day of surgery
MASTERCARD, VISA, AMERICAN EXPRESS and DISCOVER CARDS are accepted
SmartCare Palm Harbor will bill your insurance company as a courtesy, however, any balance due is the patient’s responsibility and payment will be requested from you unless your insurance company pays within 60 days
If a patient informs SCPH that he/she is not able to fully pay his/her estimated deductible, co-insurance or co-payment at the time of service, we will allow monthly payments to be made. Please contact our business office directly to make a payment arrangement
SmartCare Palm Harbor does not provide charity care. SCPH does have a financial hardship discount policy to provide financial assistance to patients who receive care but do not qualify for state or federal assistance and who are unable to pay the estimated financial responsibility in full.
Our financial assistance program offers a variety of ways to reduce a patient’s financial responsibility for services rendered by the surgery center. Our program structures a balance between offering the patient a reduced financial liability while still complying with insurance contract obligations and Federal and state regulations Please reach out directly to our business office with any questions regarding our policies on financial assistance and collections procedures.
Please reach out to your health care provider for anticipated services while at our center regarding personalized estimates, billing practices and participation with your insurance provider or health maintenance organization(HMO) as your practitioner may not participate with the same health insurers or HMO as the center.

Section 1557 of the Patient Protection and Affordable Care Act
Section 1557 is the nondiscrimination provision of the Affordable Care Act (ACA). The law prohibits discrimination on the basis of race, color, national origin, sex, age, or disability in certain health programs or activities. Section 1557 builds on long-standing and familiar Federal civil rights laws: Title VI of the Civil Rights Act of 1964, Title IX of the Education Amendments of 1972, Section 504 of the Rehabilitation Act of 1973 and the Age Discrimination Act of 1975.

Out of Network
A patient receiving treatment at our surgery center under insurance with which our facility is out of network may be eligible to receive an adjustment to their assigned out of network patient liability, assuming our facility is not prohibited from offering Out of Network adjustments under state/Federal laws or your insurance company’s provisions. If not prohibited, the application of any out of network discount is subject to vary based on a patient’s benefit coverage. Accounts which become delinquent may have the adjustment disallowed.
